Child & Family Health Services plans its activities with the goal of achieving the following outcomes:

• Monitor health status indicators
• Facilitating collaborative relationships and activities among service providers and community stakeholders,
• Increase school-focused nutrition education,
• Provide information and training about mental health needs of children and adolescent,
• Raise community awareness of the importance of childhood immunizations and screening for lead poisoning,
• Provide outreach services to pregnant women and families to gain access to Medicaid services and
• Increase and improve immunization rates, increase screening for lead poisoning and link women and children to services.
